FHMC Offers Local Non-Profit Community Grants
French Hospital Medical Center (FHMC) is proud to announce that it is now accepting applications for the 2008 Catholic Healthcare West (CHW) Community Grants Program. CHW and its member hospitals, including Marian Medical Center, Arroyo Grande Community Hospital, and FHMC, administer this annual application process to support local non-profit organizations that are driven to provide compassionate and high-quality healthcare to our community. Available grant funds total $31,500 and will be divided among several grant recipients.
The criteria for the 2008 grants will focus on the following areas: cardiac wellness; cancer prevention, resources and education; diabetes prevention and management; prenatal education and outreach to the Latino community; general healthcare education and disease prevention; and outreach to the underserved. Projects that address at least one of these areas and demonstrate the mission and core values of FHMC and CHW will be given priority. A typical grant would total $5000.
The CHW Community Grants Program is designed to help communities provide support for underserved populations. The program, which is funded by contributions from its member hospitals, is one way CHW supports and enhances the advocacy, social justice and healthier communities’ efforts of its hospitals. Some of last year’s local Community Grant recipients include the Wellness Community Central Coast, the San Luis Obispo County Child Abuse Prevention Council, and Partnership for the Children. Since the program began in 1990, CHW has granted more than $25 million to approximately 1500 organizations.
Grant materials including the Letter of Intent (LOI), Grant Application, and Program Brochure are available at http://www.chwhealth.org/About_Us/Community_Health/DV046373.
All LOI’s are due no later than Friday, August 22, 2008 by 5 p.m. After review by a local volunteer grants committee, several organizations will be invited to submit a full proposal in October, and the CHW Investment Committee will approve the final grant in early December.
