CHW Hospitals Offers Local Non-Profit Community Grants
French Hospital Medical Center (FHMC) is proud to announce that it is now accepting applications for the 2012 Catholic Healthcare West (CHW) Community Grants Program. CHW and its member hospitals, including Marian Medical Center, Arroyo Grande Community Hospital, and FHMC, administer this annual application process to support local non-profit organizations that are driven to provide compassionate and high-quality healthcare to our community. Available grant funds total $45,353 and will be divided among several grant recipients.
The criteria for the 2012 grants will focus on health education and disease prevention program areas which target those in our community with disproportionate unmet health related needs as identified in the FHMC Community Benefit Plan in the following areas: cancer prevention, congestive heart disease, cardiac wellness, diabetes, and obesity. Projects that address at least one of these areas and demonstrate the mission and core values of FHMC and CHW will be given priority. A typical grant would total $5,000.
The CHW Community Grants Program is designed to help communities provide support for undeserved populations. The program, which is funded by contributions from its member hospitals, is one way CHW supports and enhances the advocacy, social justice and healthier communities’ efforts of its hospitals. Some of last year’s local Community Grant recipients include the San Luis ALPHA Inc, the El Camino Homeless Organization, Enhancement, SLO Noor Foundation, and Transitional Food & Shelter. Since the FHMC began distributing the grants in 2006, it has given $210,111 to local non-profit organizations.
Grant materials including the Letter of Intent (LOI), Grant Application, and Program Brochure are available at www.chwHEALTH.org/communitygrants. All LOI’s are due no later than Friday, July 29, 2011 by 5 p.m. After review by a local volunteer grants committee, several organizations will be invited to submit a full proposal in September, and the CHW Investment Committee will approve the final grant in late November.
