Chronic Disease Self-Management Program (CDSMP): Healthier Living: Your Life, Take Care. This program is a free educational program for people who are 18 years old or older who have a chronic disease. It consists of series of six 2 ½ hour classes designed to improve participants ability to self-manage his/her chronic health condition. This evidence-based program was developed by Stanford University School of Medicine. The program teaches participants proven strategies to improve their capacity to live with a chronic illness. Topics include how to manage symptoms, appropriate exercise, nutrition, depression management, appropriate use of medications, communicating effectively with family, friends and health professionals and how to deal successfully with problems such as fatigue, pain and isolation. All classes are free.
Participants must attend all 6 sessions. Registration is required.
