At French Hospital Medical Center, Case Management brings the continuum of care concept to life. Case managers follow patients from admission to discharge. One of their main functions is to work in collaboration with the patient, their family and the entire healthcare team to establish an individualized treatment and discharge plan. This may include evaluating options and services that most appropriately meet a patient's healthcare needs.
The Case Manager works closely with the patient's physician and nursing staff to ensure that each patient is receiving the best level of care specific to patient needs. The Case Manager further ensures that the patient's transition from acute hospitalization to the next level of care is a smooth one. Our Case Managers are active members of our patient care staff, and can be seen addressing patient and family needs in every patient care area.
